In order to comply with the Payment Card Industry Security Standards, we regret that we can no longer accept credit card payments by mail, email or fax. PDF requests will now only be accepted by mail when paid by cheque or money order. All other requests paid by credit cards must be submitted through the Online Submission forms below or in person.
In order to complete an online submission a Netlink ID and password is required to access the order form and an American Express, MasterCard, or VISA credit card to pay the fees at the time the order is placed. People without Netlink IDs should consult the help desk or contact our office for further instructions on obtaining their Netlink ID or order transcripts in person.
The following forms are in PDF format (Portable Document Format). They can be printed off using the latest version of Acrobat Reader and then mailed or faxed to Undergraduate Records.
